PFA Navigator Impact Report

Following the completion of PACC’s PFA Navigator Project we are reflecting on the last 3 years and what an important service this has been for the SEND community in Shropshire.
PFA Navigator Service:
- Provided 1:1 support to 416 families through signposting and helping them navigate their young person’s journey
- Provided 1420 Community Support opportunities to Parent Carers and Young People through activities, training, and in-person events
- Attended 107 meetings and workstreams
- Provided over 150 in-person activities and events
- Produced 13 reports
Raised over 20 repeat themes Parent Carers have shared they are experiencing challenges with including:
- Education, Health and Care Plans
- No Education Provision
- Inappropriate Education Provision
- Lack of information on Post-16 Education Options
- Lack of information on Social Opportunities
- Mental Health support for Parent Carers
- Mental Health Support for Neurodiverse Young People
- SEND Transport
- Finances and Benefits
- Mental Capacity
- Supported Living
