New EHCP Annual Review Paperwork

PACC has been working with the Shropshire SEN Team to revise the EHCP Annual Review paperwork to bring it in-line with the EHCP template whilst trying to make it more reader/user friendly and easier for settings to complete. It is anticipated that this new paperwork will also better support a person centred approach.  The new paperwork consists of the following documents:

Agenda (This is a new document which settings can chose to use if they wish)

Views of the Child/Young person

Views of the Parent/Carer

Report template

Guidance

FAQs

Notification about this new paperwork has been sent to all education settings and all teachers/SENCOs have been asked to use this new paperwork for all Shropshire children and young people when holding reviews with immediate effect.
