PFA Navigator Impact Report

Following the completion of PACC’s PFA Navigator Project we are reflecting on the last 3 years and what an important service this has been for the SEND community in Shropshire.

PFA Navigator Service:

  • Provided 1:1 support to 416 families through signposting and helping them navigate their young person’s journey
  • Provided 1420 Community Support opportunities to  Parent Carers and Young People through activities, training, and in-person events
  • Attended 107 meetings and workstreams
  • Provided over 150 in-person activities and events
  • Produced 13 reports

 

The PFA Navigator Project raised over 20 repeat themes which Parent Carers shared they were experiencing challenges with, including:

  • Education, Health and Care Plans
  • No Education Provision
  • Inappropriate Education Provision
  • Lack of information on Post-16 Education Options
  • Lack of information on Social Opportunities
  • Mental Health support for Parent Carers
  • Mental Health Support for Neurodiverse Young People
  • SEND Transport
  • Finances and Benefits
  • Mental Capacity
  • Supported Living
