TRANSITION

  • PACC Transition Project

Thank you to all of you who responded to our request for families of young people aged 13 – 25 years to participate in our transition project. Our apologies for the delay in getting back to you, which has been due to a slight delay in identifying a project co-ordinator. We are pleased to say that we have now engaged someone to lead on this project and we will be in touch with all of you the near future.

There are many aspects to the project but its main aims are;

• To create a database of the parental experience of the transition process in Shropshire, identifying what works well and what aspects are the more challenging for families.

• To raise awareness amongst all services of the impact that the transition process can have on a family and the need for a co-ordinated approach to all aspects of a young person’s life.

• To ‘user test’ the draft Transition Pack that Shropshire Council is currently producing

On Wednesday 26th January parent representatives from PACC were present at the launch of the new Shropshire Transition Protocol which has been designed in conjunction with parents to encourage integrated working between services, throughout the transition process. At the launch Anne Lanham and Rosemary Chapman gave a short presentation describing their family’s transition experience and introducing the PACC project, to a wide range of professionals from Health, Education and Social Care below are links to these talks.

  • PACC Transition Project Final Report – The final report from the PACC Transition Project was passed to Shropshire Council on 15th July 2011. The project, funded by Shropshire Council focused on exploring the parental experience of transition in Shropshire and user testing the draft Transition Pack that is being produced to support families through the process. There was over whelming support for the Transition pack in principle, along with several suggestions for additional information that needs to be included and ideas about presentation. Transition continues to be a challenging time for families and the report highlights the need for;

 

 

1. Clear and timely information about transition to be available to families

 

2. Real choice that is based on knowledge of all the options are available

 

3. A person centred approach that recognises the individual needs of the young person and supports an aspirational approach

 

4. An understanding by all services of emotional challenges that parents face when supporting their son / daughter into adulthood

 

5. A partnership approach between service, parents and young people, where each groups voice is equally valued.

PACC will be looking to continue to work with Shropshire Council to improve the transition experience for families in Shropshire and will keep you informed of our progress.

 

  • PACC Transition Conference 2011 as part of the PACC Transition Project PACC hosted a transition conference on the 10th June in Shrewsbury. The conference was well attended highlighting the importance of this topic for families.
